Etelcalcetide is a calcimimetic drug for the treatment of secondary hyperparathyroidism in patients undergoing hemodialysis. Etelcalcetide has been approved for the treatment of secondary hyperparathyroidism (HPT) in adult patients with chronic kidney disease on hemodialysis. It is also used for treatment of pediatric hyperparathyroidism.

Mechanism of Action
Etelcalcetide is a calcimimetic agent that allosterically modulates the calcium-sensing receptor (CaSR). Etelcalcetide binds to the CaSR and enhances activation of the receptor by extracellular calcium. Activation of the CaSR on parathyroid chief cells decreases PTH secretion.

Drug Interaction

Etelcalcetide reacts severly when taken in combination with Cinacalcet. Etelcalcetide augments the effects of Cinacalcet and vice versa by pharmacodynamic synergy. Simultaneous administration of both drugs could cause severe hypocalcemia.

Adverse Reactions

Minor side effects of the medication include muscle spasms, Diarrhea, Nausea. Major side effects include Vomiting, headache, hypocalcemia, Paresthesia, Hypersensitivity, Hyperkalemia, hospitalization for heart failure, Myalgia, Hypophosphatemia, face edema, pruritic rash, etc.

Administration

The medication should not be mixed or diluted before administration. Check for discoloration or particulate matter prior to administration. The medication should be given at the end of hemodialysis. If a dosage of Etelcalcetide Drug is missed, do not administer any missed doses in between the next scheduled hemodialysis treatment.
